Parents and students,
The Thomas County Health Department is hosting a Flu Shot Clinic at Colby Public Schoolas. Flu shots will be available on Tuesday, September 26th, 2023. The Flu Clinic will be held at the Grade School from 9:00-10:00 am and then at the High School from 10:00-11:00 am.
If a student is getting the flu shot at school, the parent/guardian will HAVE to sign a consent form and return it with a copy of their insurance card and/or a form payment. Please see the attached forms for more details.
No flu vaccines will be given without parental/guardian consent.
