Enrollment Information
The Class of 2011 may pick up their diploma/transcript at the USD 315 administration building between 9:00 a.m. - 6:00 p.m.
The enrollment dates have passed; if you missed them please go to the office of your child's school and enroll there. Below you will find addresses for the buildings and a contact phone number. You can print some forms below, fill them out ahead of time and bring them with you. Please make note of the other items you will need to bring to complete the enrollment process.
- Students in Kindergarten through 5th grade will enroll in the Primary Building of the Grade School located at 210 N. Grant. Phone # 460-5100
- Students in 6th through 8th grade will enroll in the Middle School office located at 750 W. 3rd. Phone #: 460-5200
- Students in 9th - 12th will enroll in the High School office located at 1890 S. Franklin. Phone #: 460-5300
First day of school is Thursday, August 25, 2011.

If you are enrolling your child for the first time in Colby Public Schools, you need to bring the following items with you:
- Birth Certificate
- Immunization Records
- Social Security Card
You also will be asked to fill out an enrollment form, a form that requests records from your previous school,a health history form and a permission form for the website and Powerschool. You can access most of these forms below and fill them out in advance to speed up the enrollment process.

At this time the Board of Education has established the following fees. The fees will be due at enrollment.
BREAKFAST
K-12 $1.00/day ($20/mo)
LUNCH
K-2 = $2.20/day ($44/mo)
3-12 = $2.40/day ($48/mo)
Adults = $3.75/day ($75/mo)
TEXTBOOKS
$70/student
ACTIVITY FEES
$30/student -- maximum cost $90/student per school year
Students attending CPS will be charged fees for book rental, lunch tickets, woods projects (in excess of the required projects), music equipment and for any damages to books or school property. Book rental is to be paid when the student enrolls. Students moving during the school year will be given a refund prorated on the amount of time enrolled. Other fees may be assessed for activities/athletics, etc.
